The Department of Transportation (DOT) medical examination is required to legally operate a CMV (commercial motor vehicle class A, B or C) for both new applicants and renewal applicants to maintain a Commercial Driver’s License (CDL).
What to Expect from your DOT Physical?
During a DOT physical exam, your certified medical examiner will assess your health and readiness to operate a commercial motor vehicle. Areas of exam include:
- Review your health history 
- Cardiovascular and Respiratory Function 
- Vision and Hearing 
- Muscular Function 
- Urinalysis (to detect medical conditions, not for drugs or illicit substances) 
After passing your DOT Physical exam, you will be provided with a medical certificate to certify your readiness to operate a CMV.
- All applicants must come to the office with a completed Medical History Report Form (MCSA-5875) prior to their appointment. Fill out only the sections that are indicated for the patient. 
- Government issued photo ID: driver’s license, passport, or any state issued ID. 
- A list of your all your current doctors and their practice location. 
- A list of your current medications including dosage and brand name. 
- If you are taking any prescribed controlled medications, a MCSA-5895 Form will need to be completed by your prescribing physician. 
- If you wear glasses, contacts or a hearing aid, please bring them.

      An MCSA-5875 Form is a necessary form for each DOT examination. The first section, which is blue, asks about the driver’s health history, the driver must complete this section prior to the appointment.

      Make sure to show up to your appointment at least 15 minutes early and you can complete it in the office prior to the appointment.
